Learning Disabilities

As part of our commitment to reducing health inequalities for people with learning disabilities, we now have 2 Learning Disability Champions at the Practice.

Dr Jack Middleham and Emma Little are our named Learning Disability Champions and are being supported by the LSCFT Health Facilitation Team.

The role includes:

• Increased Learning Disability awareness for practice staff
• Increase in positive experiences of accessing Primary care for patients and their carers including ensuring reasonable adjustments are in place.
• Improved uptake of Annual Health Checks and vaccinations.
• Demonstrating the Practices commitment to working with vulnerable patients with a focus on Learning Disabilities
• Correct implementation of the Mental Capacity Act 2005, Accessible Information Standards 2016, Equality Act 2010
• Demonstrate collaborative working with the Learning Disability Health Facilitation team to improve health outcomes.
