Frailty assessments

All GP practices in Lancashire & South Cumbria have been tasked with proactively looking at patients over 50 years of age to assess whether they need any assistance with everyday tasks.

The electronic Frailty Index (eFI) is a tool that uses health record data to automatically calculate a score that indicates a person’s frailty level. The eFI is based on a cumulative deficit model that considers 36 deficit variables, including clinical signs, symptoms, diseases, disabilities, and abnormal test values. The score is calculated by dividing the number of deficits present by the total of 36. The eFI can be used to identify people who are likely to be fit or living with mild, moderate, or severe frailty.

The eFI is not intended to be used as a diagnostic tool, but it can be used to identify people who may benefit from evidence-based interventions.

PSM Medical Group has around 4,000 patients who are 50 and over, so we use the electronic frailty index (eFI) to try and identify those who are most likely to need a more detailed ‘Over 50’s assessment’. Therefore, all patients over 50 will have had an eFI score applied to their medical record, which you may see if you view your records online.
